NEW Requirements for 7th & 8th grade students

New for 2013-2014 school year: 1st Day Exclusion Policy

All physical & immunization records must be submitted PRIOR to the first day of school, August 16, 2013. Forms may be brought to registration or dropped off/mailed to the school, attention: School Nurse

Failure to comply with the physical & immunization requirements by the first day of school (August 16, 2013) will result in your child being excluded from school until such requirements are met.

Preschool Students-First year

• Completed Illinois School Physical Form  due prior to the 1st day of school 
• Proof of current immunizations and/or waiver due prior to the 1st day of school 

Preschool Students- Returning- no additional health requirements

Kindergarten Students

• Completed Illinois School Physical Form. Due prior to the 1st day of school 
• Proof of current immunizations and/or waiver. Due prior to the 1st day of school 
• Completed Eye Examination Form or waiver. Due by October 15, 2013
• Completed Dental Examination form or waiver. Due by May 15, 2014

2nd Grade Students

• Completed Dental Examination form or waiver. Due by May 15, 2014

6th Grade Students

• Completed Illinois School Physical Form.  Due prior to the 1st day of school 
• Proof of current immunizations and/or waiver INCLUDING proof of Tdap dose given after age 11. Due prior to the 1st day of school 
• Completed Dental Examination form or waiver. Due by May 15, 2014

• Updated Immunization Records and /or waiver INCLUDING proof of Tdap dose given after age 11. Due prior to the 1st day of school
